Breast filler enhancement: what to expect before and after

The revolutionary breast filler injection has made breast enhancements such a breeze, compared to the past where the only hope for sizeable breast enlargement was through a drastic process of surgically inserting a silicon implant. There are a few reasons why breast filler injections are proving to be more acceptable to our patients. Firstly, compared to silicon breast implants, there is no need to undergo general anaesthesia, which itself carries a certain degree of  unnecessary and some even consider unacceptable risk to the patient. Secondly, the scar that breast silicon implants leave are not ideal in a society that still has not accepted this form of cosmetic enhancement in women. In contrast, breast filler injections leave a 1mm injection mark that usually fades with time in the majority of patients. Thirdly, the look/feel of silicon implants are also not as natural as they should be. Breast filler injections on the other hand have been described to feel very natural, making it difficult to differentiate between real breast tissue and injected material. Finally, as the filler material are biodegradable and non-toxic, there is no need to remove the filler later-on. In the case of silicon implants, the manufacturers usually only warranty their implants for 10 to 15 years, meaning that the patient will need to consider removing the implant years down the road.

Sizing it up:

Breast filler injections are ideal for ladies who want about 1 to 1.5 cup size increment. Before we go on, what exactly is a cup size? It is generally determined by the difference in inches between the circumference of the breasts measured at its maximum (usually at the nipple level, rounded UP to the nearest inch) and the circumference just below the breasts or the infra-mammary fold. Breast cup size is as follows:

AA cup: 0 inch

A cup:    1 inch

B cup:    2 inch

C cup:    3 inch

D cup:   4 inch

Cup size is only a rough guide to the actual size of the breasts. This is because the measurement method depends on the breast size, relative to the actual girth of the chest. For example, if a lady is very petite or small framed with moderate sized breasts, her breasts cup size would be relatively larger than a lady with the same breast size but a bigger or wider frame or more surrounding fatty tissue. What is most important in assessing how attractive breasts are, as determined in independent studies is the shape of the breast and how it is in harmony with the rest of the woman’s shape and curves. With that in mind, it is very important to know what breast shape will suit a particular body frame.

Breast filler injections are designed to be mouldable for that reason. With skill and proper training, and a good aesthetic eye for beauty, a doctor can bring out the best in what many consider to be a woman’s most important assets. Be it symmetry, or nipple height and projection or correcting inborn deficiencies in the breasts, all these will play a part in achieving what many doctors fail to appreciate and hence lead to less than perfect results.

At Freia, we believe all the above applies in our assessment of our patients. We listen to our patient’s expectations, needs/wants, do our objective assessment including a pre-procedure ultrasound of the breasts in order to ascertain health and suitability for the procedure. In less than a week from the first consultation, the filler injection can be carried out and we normally allocate about 2 hours for the whole process. This ensures that neither the patient nor our staff are rushed to complete the procedure before the next patient arrives. Our patients are typically advised not to drive on that day, as some medication may interfere with her ability to operate machinery. In addition, she is reminded to bring a non-underwired bra, a sports bra or bikini. This avoids causing any indentation onto the breast filler before it “sets” over the next 24 hours. She is also advised not to engage in any strenuous activity for a week. In addition, she will be given a course of antibiotics and painkillers. She will feel mild chest soreness post-procedure, which is likened to a heavy work-out at the gym. Finally, she is given a return appointment one week later for a review. In most instances, our patients return one week later only to report that they are very happy with the outcome. They usually report that as the initial swelling subsides, their breast size will go down about 10% compared to immediate post-op. As a general reminder, they are encouraged to do their monthly breast self examination, looking for any unusual lumps or changes to their breasts.

The more the merrier

Some of our patients return after a few weeks or months to request for a “top-up” of the filler for further enhancement. This can be done as early as 4 weeks after the first procedure.

There are 2 main filler materials used in Singapore and across the world today: namely Macrolane, a hyaluronic acid synthetic and Aqualift, a non-toxic polyamide. They are both manufactured in Europe with the CE mark of safety and standard. Macrolane lasts about 1 to 1.5 years while Aqualift lasts between 3 to 5 years.

Both have a very natural look and feel and sometimes, even experts find it hard to distinguish between  a natural bosom from a filler-enhanced one.
